Specialty-scoped NPI lookup

NPI lookup for pharmacies

The CMS NPI Registry finds pharmacies through the taxonomy on the record, using taxonomy_description=Pharmacy. That parameter works nationwide on its own and is also the only way to scope a state, because a state-only query is rejected with error 07. The records below come from a live call to the official API.

Pharmacy records from the official registry

Last verified: 2026-07-30. date_retrieved: 2026-07-30. The registry matches the taxonomy description as a prefix, so a search for “Pharmacy” also returns longer classifications that begin with it. Each row shows the description the registry returned for that NPI rather than the search term.

Up to 10 records from a response CMS caps at 200. This is not a complete list of pharmacies, not a ranking, and not a credentialing, licensure, or exclusion-screening result.

What taxonomy is

Taxonomy is classification, not credentialing

The value in a registry record is a NUCC Health Care Provider Taxonomy classification. Providers select it during NPPES enumeration and maintain it themselves, so it describes how the record is classified rather than a verified scope of practice. NPI issuance does not validate licensure, credentialing, Medicare enrollment, or current practice authority.

A record can also carry more than one taxonomy. The table above shows the primary taxonomy where the registry marks one, and otherwise the first taxonomy on the record; the full set is on each NPI profile.

Narrow by state

Pharmacy NPI lookup by state

Adding a state to this taxonomy is the query the registry accepts. Each state page runs it and shows the result with the official URL behind it.

The Pharmacy records on this page are read live from the official CMS NPI Registry API and re-checked on each rebuild. SourceCheckHealth does not add, edit, or rank registry records, and taxonomy is CMS classification rather than licensure or current practice authority.

FAQ

Practical notes

Is this every provider with this taxonomy?

No. A single CMS NPI Registry API response is capped at 200 records, so the rows here are a dated sample of one taxonomy query. Use the official registry search to page further, and treat this page as scope guidance rather than a directory.

Why do some rows show a longer taxonomy description?

The registry matches taxonomy_description as a prefix, so a search for "Dentist" returns records classified "Dentist, General Practice" and similar. Each row shows the description the registry actually returned for that NPI.

How do I narrow this to one state?

Add a state to the same query. A state-only search is refused by the registry, but state plus taxonomy_description works, and the state links on this page open pages built on exactly that pairing.

Does this replace a credentialing check?

No. Registry taxonomy is classification, not licensure, credentialing, Medicare enrollment, or current practice authority, and it carries no federal exclusion status. Screen identifiers against the OIG LEIE separately.
