Use public-provider data carefully
SourceCheckHealth is built for public provider, organization, and lab screening. Avoid uploading patient data, protected health information, medical records, or unnecessary personal details.
Submitted search data
Lookup and CSV screening inputs may include public provider identifiers, names, organization names, states, ZIP codes, and CLIA numbers. Screening runs are saved so users can review and export results.
Operational records
The application stores screening results, import snapshots, source checksums, and basic operational metadata needed to run the product and explain what data was screened.
Sensitive information
Do not submit PHI, patient identifiers, clinical notes, Social Security numbers, payment card data, or private employment records. The tool does not need those fields to screen public compliance sources.
Verification
Search results are derived from public datasets and may be incomplete, stale, or ambiguous. Always verify against official sources before taking action.
