Verified against the official registry API on 2026-07-30, a state-only search is refused with error 07, “Field state requires additional search criteria,” and any single response returns at most 200 records. So there is no free official way to list every NPI in Texas or any other state; a state has to be paired with a specialty, city, postal code, or name.
