Last verified: 2026-06-13. date_retrieved: 2026-06-13. CMS points lab demographics lookup users to QCOR and publishes Laboratory Registry information for public review. Use the official CMS QCOR lab lookup to confirm current certificate context before acting. SourceCheckHealth also imports CMS Provider of Services clinical laboratory rows when available.
Review fields
What the lab page can show
- Lab identity: CLIA number, lab name, city, state, and ZIP when the imported CMS row is present.
- Certificate/status: imported certificate or status fields when included in the local CMS clinical laboratory source.
- Verification path: official CMS/QCOR links remain visible because SourceCheckHealth is not a final CLIA certification, credentialing, or compliance decision.
Certificate scope
The five CMS CLIA certificate types
A CLIA number is the lab's federal identifier; the certificate it carries defines which test complexities the lab is authorized to perform. A “CLIA license lookup” is really a lookup of the lab's CLIA certificate— labs are authorized under a CLIA certificate, not a separate license. CMS issues five certificate types under the CLIA program. Last verified: 2026-06-13. date_retrieved: 2026-06-13.
- Certificate of Waiver (CoW)
- Authorizes a lab to perform only CLIA-waived tests.
- Provider-Performed Microscopy (PPM)
- Covers specific microscopy procedures a physician, midlevel practitioner, or dentist performs during a patient's visit, plus waived tests.
- Certificate of Registration (CoR)
- Allows non-waived (moderate and/or high complexity) testing until the lab is surveyed for compliance.
- Certificate of Compliance (CoC)
- Issued after a State Agency or CMS survey finds the lab compliant with applicable CLIA requirements.
- Certificate of Accreditation (CoA)
- Issued on the basis of accreditation by a CMS-approved accreditation organization.
The imported CMS clinical laboratory row shows only fields present in the source file. For the current certificate type, status, and any expiration context, verify the live record in CMS QCOR — SourceCheckHealth is not a final certification or compliance decision.

Practical notes
Why is CLIA marked limited?
CMS QCOR may be unsuitable for automated ingestion. This MVP links to official lookup unless reliable structured data is imported.
Can I use this as a CLIA certification lookup?
Use it to review imported lab identity, certificate/status fields, and source import time, then verify the current certificate context in official CMS QCOR before acting.
Does this show a CLIA certificate expiration date?
Only fields present in the imported CMS clinical laboratory row are shown. Open CMS QCOR for current certificate details and any fields not imported here.
Is a CLIA license lookup the same as a CLIA certificate lookup?
Yes. Labs are authorized under a CLIA certificate rather than a separate license, so a CLIA license lookup means reviewing the lab's CLIA certificate type and status. Use the imported CMS identity and certificate/status fields, then confirm the current certificate in official CMS QCOR.
How do I look up a CLIA number?
Enter the lab's CLIA number to open the matching record with imported CMS identity, certificate/status fields, and source freshness. CMS issues five certificate types (Waiver, PPM, Registration, Compliance, Accreditation), and the certificate type defines which test complexities the lab may perform. Open the official CMS QCOR lookup for current CLIA number verification before acting.
